Whooping cough: symptoms of the disease in children and treatment

Does the baby cough for a long time and specifically, and no antitussive medications relieve his condition?Perhaps the child has one of the most common childhood diseases - whooping cough. What is this disease, how it is diagnosed, and what needs to be done to cure the baby, we will tell you today in our article.

Features of the disease

Whooping cough - This is an acute infectious disease with lesions of the respiratory tract and attacks of spasmodic cough.

Causative agent- pertussis wand, unstable to the external environment, causing the breakdown of red blood cells.

Sources of infection- A sick person transmits pertussis wand by droplet through close and sufficiently long contact. It is impossible to get pertussis through objects.

Most often, whooping cough are sick preschoolers, especially in autumn and winter.

Periods of illness:

  • incubation period - 2-14 days,
  • the initial or catarrhal period is 2-14 days,
  • the period of convulsive cough - from 1 month or more,
  • recovery - 1-2 months.

After the pertussis stick gets on the mucous membrane of the upper respiratory tract, in the process of vital activityit releases a toxin that has a negative effecton the respiratory, vascular, nervous and immune systems of the baby. As a result of this influence, the fibers of the vagus nerve are damaged and the transmission of nerve impulses to the respiratory center is impaired. Calcium metabolism and blood supply of the child’s central nervous system are disturbed, a tonic spasm of the respiratory muscles arises, an attack-like cough appears, whichit is important to recognize and consult a doctor.

Our mother-forumchild Irbis tells:“The main difference between an ordinary cough after a cold is whooping cough — whooping cough is specific enough, with a reprise, and a good doctor will determine that it is whooping cough, even after hearing a child cough. The insidiousness of whooping cough is precisely that this characteristic cough develops gradually - it is difficult to describe: several pushing coughing attacks, then a deep breath (reprise) - and again the same pushing that can last for several minutes.Such a cough is associated with the fact that the pertussis toxin affects precisely the nerve endings, as a result of which a constant irritation of the cough respiratory center occurs. Although our doctor said that she had several times encountered practical cases of the absence of a characteristic reprise — just a sore throat and cough tremors, the diagnosis was later confirmed in the laboratory. ”

Important nuance: The most dangerous whooping cough for children aged 0 to 12 months. Their susceptibility to whooping cough is very high from the moment of birth, since immunity to this disease is not transmitted to the child from the mother. In cases of severe coughing in babies, breathing is possible, and in such cases, children must have inpatient treatment.

Koval Oksana Anatolievna, pediatrician of the Dobrobut Children's Clinic tells: “For the diagnosis of pertussis, the following tests are necessary: bacteriological seeding with the release of Bordetella pertussis pertussis stick. Sowing is desirable to take in the first 5 days of the disease, the result of the analysis will be ready on the 7th day. If tests show that a child has whooping cough, treatment is prescribed, most often it is antibiotics, the period of administration is 7-10 days, it is also possible to administer antitussives. In severe forms of the disease and if a whooping cough causes a child under the age of 1 year, the patient must be hospitalized, due to the fact that if the child does not receive appropriate treatment, he is threatened with respiratory failure, whooping cough pneumonia, convulsive syndrome may occur during hypoxia brain".

How to recognize whooping cough

More oftenknowing the disease right away is very difficultIt begins as a mild cold with a low temperature.runny noseand cough. Graduallytemperaturethe child is stabilizing, he feels well, a runny nose passes, only a cough remains. In this situation, the child often begins to go to school or kindergarten, which is very sad, because whooping cough is most infectious during the catarrhal period. However, parents can only suspect that the child has whooping cough, and not just the effects of a cold, only if the child has had contact with the patient.

It would seem that the cough will gradually pass, but this does not happen - the child continues to cough strongly, and no medications give a visible result. The attack of dry spasmodic cough, interrupted by a deep whistling breath - a reprise, and can end with vomiting or separation of a thick transparent "vitreous" sputum. The child often coughs with attacks, gasping, while walking, his condition improves, the attacks reach their maximum by the end of the second week of the disease, then, most often, the cough subsides. In general, the period of convulsive cough lasts 3-4 weeks, but it happens that the seizures persist for 4-6 months.

Our mother forumchankaJuliko tells:“Whooping cough is an insidious disease, we cough for a month and were treated until the clinical picture clearly emerged. Initially, it proceeds without any symptoms, cough, runny nose, the temperature may or may not be, for example, we did not have. Its only noticeable difference from the usual acute respiratory viral infections or acute respiratory infections in the fact that you treat, treat cough, but it not only does not pass, but it gets worse. After 2 weeks of normal cough, we had a very strong dry cough, fits, and it gradually increased. During the third week, during an attack, my daughter was blushing, sometimes the attack ended in vomiting. At night, too, bouts, even worse. "

Therefore, if after the usual coldbaby cough does not passand vice versaintensifiesfollowsbe sure to consult a doctorfor the diagnosis of whooping cough.

Catherine Kovbasko, pediatrician clinic "Dobrobut" tells: “A characteristic cough with whooping cough does not form before 3-4 weeks after the onset of the disease. For accurate verification of the diagnosis of pertussis, as, indeed, any other infectious disease, it is necessary to take the so-called "paired serum", i.e. repeated antibody tests after 2-6 weeks to track the dynamics. Remember that any disease has its own specific nature of development, it also matters whether the child is vaccinated. Considering all the above factors, if the cough does not go away for a long time, you should trust the doctor and together identify the cause of the cough. "

Diagnosis of the disease

Of course, an experienced pediatrician can diagnose whooping cough during the examination, but sometimes the picture of the disease is blurred, unclear, and in order to make the correct diagnosis, it is necessary to conductlaboratory research.

If you go to a doctor at an early stage of the disease - until the second week of the spasmodic cough period - and did not take antibiotics during the treatment, the child can be sent to be sown on special media from the back of the throat. In the case of antibiotics, this method is not informative.

Also, a child with suspected pertussis will need to donate blood for the concentration (titer) of antibodies to the pertussis stick. The greatest value in this analysis is the increase in the titer of specific antibodies with repeated studies. Also, according to the blood test, antibodies of the immunoglobulin M and G class to the pertussis pathogen, an increased number of leukocytes and lymphocytes in normal ESR are determined.

Pertussis treatment

As we have said, babies under the age of 1 year should be treated for whooping cough in a hospital setting, and such treatment is necessary for those children who have developed complications after whooping cough. In other cases, the child is treated at home.

Most often, for the treatment of the disease, doctors prescribe antibiotics, anticonvulsants, antitussive and expectorant drugs, sedatives, homeopathic medicines.

Pediatrician Maria Savinova tells: “In the treatment of whooping cough, the main thing is patience. From my own experience, I can say that it is impossible to cure whooping cough in a few days, but homeopathy helps to soften the attacks, reduce their frequency and shorten the period of spasmodic cough. If patients come to me with a cough that lasts for more than 2 months, homeopathy can cope with it completely and in several doses of the medicine. ”

During treatment, the child needsgentle mode with mandatory walksin the fresh air, balanced dietand fascinatinginactive gameswhich will switch the focus of excitation of the baby in the respiratory center of the brain, thereby facilitating the child's condition. During treatment, it is important to create a situation for the child in order to exclude all possible agents that cause pertussis cough, because coughing attacks can provoke exercise, sneezing, chewing, swallowing, and negative emotions. Also, do not forget to air the room in which the little patient is located as often as possible, because the fresh, cool air favorably affects the speed of his recovery and combats the dust.

Prevention and Vaccination

A patient with whooping cough at the age of 7 years should be isolated from other children in the house whenever possible; if this does not work out, prophylactic medication and bacteriological examination of the pharynx is recommended by a doctor.

Specific prevention of whooping cough is providedvaccination. In Ukraine, pertussis vaccination is carried out from 3 months of life up to 4 years three times with a minimum interval of 30 days. Revaccination is carried out once at the age of 18 months (6-12 months after the third vaccination). If a child under 4 years of age has not received revaccination with DTP vaccine (whooping cough, diphtheria, tetanus), then it is performed by DTP (without the pertussis component for children 4-6 years old) or DTP-m (with a reduced number of antigens) for children over 6 ti years. The pertussis vaccines are divided into whole-cell - DTP and acellular - aAKDS.
